People living with diabetes across Cheshire and Merseyside can now register for ‘MyDiabetesMyWay’ an online platform to support them in improving how they self-manage their condition from home.
In our region there are around 186,000 people who live with diabetes and through the MyDiabetesMyWay service, they’ll be able to access a range of multi-media educational resources including videos, leaflets and eLearning courses for free when they register.
The MyDiabetesMyWay service launched this month and was funded by the Cheshire & Merseyside Health & Care Partnership, which is a body of NHS organisations, clinical commissioning groups, local authorities, voluntary, community and faith sector organisations. The Partnership were awarded funding by NHS England and Improvement to develop the online platform to support their role in working collaboratively to make sure that everyone in the region has access to better, more joined up, sustainable care, to stay healthy and live longer. The online platform will support the Partnership in this duty, by empowering diabetes patients in the footprint to take control of and manage their own health and wellbeing through the MyDiabetesMyWay service, which is available free of charge to them.
Sarah O’Brien, Senior Responsible Officer for Cheshire & Merseyside Health & Partnership’s Diabetes Network, said “A lot of information is available online about diabetes, but for patients it’s not always clear where they can access resources that have been recommended or approved by health and care professionals ensuring it’s factually correct, up to date and safe to use. The MyDiabetesMyWay service highlights how we can use digital technology to combat this issue, as it provides those living with diabetes in our region with access to clinically evaluated information about their condition, so they can understand it better and feel more confident in taking ownership of and managing their health and wellbeing outside of a clinical setting, and at a time and place that is convenient for them.”
People in Cheshire and Merseyside living with diabetes can utilise the MyDiabetesMyWay service and all it has to offer, by registering on the site itself at: https://cm.mydiabetes.com/register
- The Cheshire & Merseyside Health & Care Partnership was created in 2016. It was established to confront the health and care challenges in Cheshire and Merseyside of population health, the quality of care, and increasing financial pressures.
- The Partnership is not an organisation but a collection of all the public services that provide health and care in Cheshire and Merseyside – the NHS, GPs, local councils and the community and voluntary sector – coming together to plan how best to deliver these services in future so that they meet the needs of local people, are high quality and are affordable.
